    Can you please explain why my 89 year old mum that lives in Scotland , when she needed a hip replacement went private as the nhs waiting list was so long. It cost her £16000? Despite no complications. I live in Australia. A friend whose father in law retired out here needed the same op and had it had done privately. Again no complications but it cost $18000. That’s in uk £16000 =Au$ 31000 Australia Au $18000, A huge difference. But the care is every bit as good if not better in Oz and health care professionals get better pay. So the uk is making a killing if you excuse the unfortunate pun! Such greed! Just how much is enough? Now retired I was a double qualified RN having trained I the NHS and emigrated 30 years ago. When I got my first pay check I actually thought I had been overpaid. But it turned out I wasn’t . So better pay for working less hours. I thought all my Christmas’s had come at once.This loss of health care professionals is only going to get worse unless drastic action is taken by government in the uk. I also heard recently that uk trained nurses may have to upskill as their competencies are falling short of Oz equivalent!! Never in my wildest dream would I thought that would happen. And yet still people still say that theNHS is the best in world , but the reality is nothing could be further from the truth.

    I think people should really consider coming to Germany! I moved out of UK and the NHS Hell! I finished my residency in germany and during my residency I got a decent pay. I made a starting salary of around 70K+ and in the last years about a 100K+/year. And after residency a lot more options with better pay are available! The german healthcare is a huge markert for doctors and you can almost get into any speciality of choice. You just have to learn German to B2 level. The work life balance can be managed pretty well but of course it isn't all Hunky Dory. And if you get sick of Germany, you can just move to Austria or Switzerland.

    I moved out of the UK after F1 and moved to Germany and I finished my residency a year ago. When I started my residency back in 2017 I made an average salary of just above 70k Euros (Gross) as a first year resident. I think I worked about 46 Hours on average. I did Radiation Oncology as a Specialty. Over the period of about 6 year it went up to 100k+. Post Residency its a lot better and working hours in Private Practice are like max 42 hours a week. Obviously, it varies .. some slightly senoir colleagues make 200k/year for 42 Hour week. GPs in underserved areas make also very good money. Locums in Emergency Med or Pysh or Radiology pay also really well. I know some people who work about 45 Hours doing locums with a base salaray of 14k a month plus extra depending on the sessions. And the jobs are Endless.

    Great points doc. I really want to practice medicine in Australia but ik plab is much easier than amc and they offer good opportunities to young doctors and freshers so i wanna give plab and work for 2 years and then move to Australia. In that way it will be easier to get job in Australia too. Does that sound like a good idea doctor. Pls give your suggestions

      @@DoctorKenji Also high earners pay extra 1.5% surcharge if they use public healthcare. This encourages them to take out private insurance instead. It's worth noting that 50% of australians have private insurance and that reduces burden on the public sector, improves working conditions and wages. British public is extremely opposed to private sector.

      @@esvedra2419 agree. I trained in the NHS as a double registered nurse and moved to Australia in 1989. Old school lol.At aged 38 I suffered a major stroke but wasn’t considered a high risk so they left no stone unturned in testing to find out the underlining cause which was a minor congenital heart defect. I lost the ability to talk, walk, swallow, balance, read, spell, count as well as the use of my right hand. With much support I rehabilitated to the point that the only residual issues I had were the non use of my right arm and hand and slight balance issues. I even found employment in tertiary education. I’ was treated as a public patient in a public hospital and could not fault the care I received. It didn’t cost me a thing.People in the uk have a blind spot when it comes to health care , and the nhs is unsustainable in its current form. Health care world wide has become very expensive but if people were made to contribute a bit more towards it then maybe they wouldn’t be so wasteful .Out here its means tested . A mix of public and private seems to work and the nhs needs total reform but right now I don’t see anyone with the underpinning background, skills , political will and the guts to successfully undertake the challenge. Wes Streeting sure has his work cut out for him.

    Why do students have to achieve higher grades to get into the medicine when there is a possibility to work like doctors by getting lower grades? Why wasting too much time to become a doctor when one can do it with less effort? If diagnosis and treatment are so easy, then why not ask patients to treat other patients? Google and RUclips are producing many doctors. No matter what, I want to be diagnosed and treated by doctors. I have nothing against other health care professionals because they are part of the health care system. LET THE PILOT FLY THE PLANE AND NOT THE CABIN CREW FOR SAFER JOURNEY.

    If the intention was to make the service more efficient then I'm sure no one would argue with that. But if the intention is to cut funding then that's a different thing all together. 'Sacrifices' will always be made. In this case, it is your health. Efficiency is the government's euphemism for cutting funding.

      It is 48 hours a week averaged across a month. Sometime we work 7/8 days straight. Other days we only work 4 days. It varies.

    Starters position in a supermarket in the Netherlands here earn an hourly rate of €18,- , roughly 15.5 pounds. I think 17 pounds is ridiculously low for a university lvl employee tbh PS, u earn double or 150% when working on Sundays and holidays or late hours(IN SUPEEMARKETS!!!)

    The most ignored group in this muddle are pharmacists .I suggest strike and close down whole stores in the UK.Every week there are 10 chemists closed in the UK and wages are going down due to store closure,more stress and every pharmacy is becomming more busy .This is not true that doctors are more responsible.The prescription is a document but I had a doctor who wrote on piece of paper for the patient how to take medicine .The instrution was too compliateted so practisioner wrote as directed for the patient fit .This is an example how doctors are far from reality .He precribed for the patient medicine and every day was different .He should have prescribed than weekly prescription or slighly simplify .This is an example how doctors slipped away from responsibility .Prescription used to be important document .Pharmacists won't stand in the chemist anymore.

    With all respect but please look into the requirements for applying for physician associate studies, there is no medical school in the country which will accept a student with a history degree no way I challenge you on this in the nicest possible way

    I agree with everything here. There could be use for the PAs such as taking measurements or readings but certainly not diagnosing. Also they should probably change the title to physician assistant.

      @@AtifKhan-ir7oe Australia doesn’t have PAs and the country is doing fine. In fact more than fine, its patient safety record is miles ahead of the UK and has a much better medical system than the UK. Same with New Zealand. You don’t need PAs to provide good healthcare, you NEED doctors.

      @@varadillath4508 Like comparing apples to oranges. Australia doesn't have a national health service and operates completely differently from the UK. If training more doctors was the answer, then we would have the best healthcare system in the world. Lack of training places and poor retention is the problem. Attacking other professions does not help solve the problem, these roles are here to stay and yes they have their flaws but creating toxic environments in healthcare doesn't help solve anything.

    Doctors hate them because PA are crap. I am talking as patient. No a person with a History degree cannot do a MA to become a PA. A person needs to so a science degree first etc. Medical centres do not tell patients they are seeing a PA and the latter does not either. In fact they pretend they are doctors and give that impression to the patient, this is illegal as impersonating a doctor is against the law. PAs are lacking and you can tell as you walk in. Doctors and Nurses have to know their stuff, so they spend a long training for it, this important as the public will pick up on lack of knowledge as it translates into lack of confidence. A PA with both is damn dangerous but a doctor and nurse usually are very confident as they are well trained for years and have status. PAs have no status other than a confusing moniker. PAs do not provide access to health care, as they act as a barrier, who do not take symptoms of a patients esp. when it comes to mental health seriously. When i asked for see a doctor, i was given a PA instead, i was not told the person was a doctor. When i went in the room I assumed they were a doctor, I picked up something was wrong with the PAs mannerisms, so i asked if they were a doctor and they said no. I was shocked. Now i lack confidence in the medical services provided, it was rock bottom already due to DWP medical assessments and the way doctors treated me over my Mother's death and the stitch up at the inquest. I never in my life thought the medical profession would pass an unqualified person as doctor. When i was seen by a PA there was no doctor in the room and the former had already made a diagnosis and just got the doctor to sign the prescription. No doctor saw me, that is wrong, imagine if i was given the wrong drug, or treatment, then i had a reaction to it and that cost me my life. It would not be the PA that is at fault but the Doctor who signed the prescription. My final thought PAs as assistants to doctors is a confusing status, nurses are there to be assistants to doctors and provide basic medical care. If you have PAs who are not nurses, who see themselves as more akin to doctors that is a problematic. As i have said nurses and doctors are for better trained but you if you have another role that is less trained but have a higher status than a nurse, the government is filling the NHs with pseudo medical staff.

    Doctors need to learn to be better leaders, practice teamwork skills, and most importantly, agree to supervise PAs. Doctors that refuse to cooperate with PAs are the problem. It's not like PAs need any of this extra drama.

      @@esvedra2419 why not? Is caring about money bad? Especially since people are struggling to live in places like London especially with the current inflation crisis. Jobs which have higher risks and higher responsibility are paid accordingly. That’s how the world works. Because the UK went Soviet Union style by socialising almost every bit of the UK healthcare, it’s needed up slashing doctors salaries by half. That’s one of the reasons why no dentists work in the NHS anymore and on average why they earn more than doctors, it’s because they work privately, not because their job is harder or require more responsibility. In almost all other developed country, medical specialists earn more than dental specialists. I am a staunch believer in if you work hard, sacrifice a bit of your social life and put effort into school, then you should be rewarded for that sacrifice. If you don’t believe in that then you believe in communism. Or you’re just a free loader.

    What's wrong with doctors? Even in USA doctors I hear they do not like the Doctorate Nursing Practice DNP. They forget that A mathematicians or psychologists they can be called doctors if the individual achieved a doctorate degree.

      Those arent medical doctors and cannot use the title doctor in a medical setting.DNP isnt even a clinical qualifocation ,its often about education and used for lecturing or leadership roles.

    Locum market is dying mate for shos, I've done sho locum years and reg locum years. I'd strongly suggest taking time out once you've completed core.
